Respecting your right to choose …

As a psychiatrist, I often prescribe medications to help people with serious mental health issues that may be causing problems in their life. Some patient welcome the use of medications, some are hesitant but hopeful, while others do not want any pharmacological intervention. I do my best to provide as much information and education about the use of medications including the potential side effects, expectations of benefits and how they work. I offer my recommendations and work with the patient to decide the next step. I’ve always thought of what I do is to enhance the quality of life of the patient by decreasing stress, improving mood and functioning and making life more enjoyable. Still, the decision for most individuals as to whether or not to take medications lies within oneself. Interestingly, many people who refuse the help of medications find more destructive ways to self-medicate through alcohol, illicit drugs or other impulsive behaviors that cause even more problems in life. I often let my know that there are side effects to NO medications when a significant mood disorder is present. Stress cause the body to over-produce the hormone cortisol which can be neurotoxic to the body and stress can also decrease the production of naturally produced neuroprotective substances such as brain-derived neurtrophic factor (BDNF) which protects neurons in the central nervous system. In the end, the decision still is with the patient to select the treatment they feel is best for them and the physician should be there to respect that decision and continue to provide education, options and support. – Dr. Jeffrey DeSarbo
